I am voluntarily wishing to experience a session of Therapeutic Massage/Reiki by Becky Hudgins, CMT (certified massage therapist) CRP (certified reiki practitioner).
I understand that massage therapist / reiki practitioners do not diagnose illness, prescribe medications, or make spinal adjustments. I further understand that massage / reiki is not a substitute for medical care or treatment.
I have alerted my therapist / practitioner to any conditions I have which may affect the work and have disclosed all medications (herbal and/or pharmaceutical) that I am currently taking. I further agree to update my practitioner to any changes in my mental, emotional, or physical health.
I am seeking therapeutic massage / reiki of my own accord for the purposes that massage / reiki is intended. Such purposes include but not limited to: relaxation, mental wellness, relief of sore muscle tension, improved circulation, and/or improved range of motion.
I understand if there is any inappropriate behavior (sexual advances or sexual talk) the session will be terminated and the session is to be paid in full.
Cancellation Policy
Please understand that your time commitment begins at the moment you reserve a session. Consider your schedule carefully and do not make an appointment at a questionable time. However, I realize unexpected events happen and a cancellation may be necessary.
Because last-minute cancellations cause conflicts and financial difficulties, payment is expected unless a 24-hour notice is given via text or phone message. Thank you for your understanding in this matter.
