I am voluntarily wishing to experience a session of therapeutic massage by Becky Hudgins, CMT. 

I understand that massage therapist do not diagnose illness, prescribe medications, or make make spinal adjustments.  I further understand that massage is not a substitute for medical care or treatment. 

 

I have alerted my therapist to any conditions I have which may affect the work and have disclosed all medications (herbal and/or pharmaceutical) that I am currently taking.  I further agree to update my practitioner to any changes in my mental, emotional, or physical health. 

I am seeking therapeutic massage of my own accord for the purposes that massage is intended.  Such purposes include but not limited to: relaxation, mental wellness, relief of sore muscle tension, improved circulation, and/or improved range of motion.  

I understand if there is any inappropriate behavior (sexual advances or sexual talk) the massage will be terminated and the massage is to be paid in full. 

Cancellation Policy

 

Please understand that your time commitment begins at the moment you reserve a massage.  Consider your schedule carefully and do not make an appointment at a questionable time.  However, I realize unexpected events happen and a cancellation may be necessary. 

Because last-minute cancellations cause conflicts and financial difficulties, payment is expected unless a 24-hour notice is given via text or phone message.  Thank you for your understanding in this matter.
